I hope that you have acquainted yourself with the basics of different visa types. At most US institutes you can do an elective/clerkship as an international medical student on either B1, F1 or J1 Visa. Specific pros & cons for each of these visa types for purpose of doing Electives, as well as details about application procedures & discussions about specific rules & reguations have been been elaborated in extensive details under USA VISA & Elective/Clerkships. I advise you to go through that article as well, I am sure you will find useful information.

Different US based Medical Universities & Hospitals that accept medical students for elective rotations have specific regulations with respect to VISA policies. Provided that you have been accepted for an elective rotation, MOST places will accept you to participate in their Elective/clerkship program under B1 Visa only. Others will accept you on F1 only, a few on J1 only & a handful are willing to accept you on either & will sponsor your Visa as required.

Remember that B1 visa is NOT sponsored by the University, you obtain it on your own at your local counselate based on the Acceptance letter alone. On ther other hand, If the University is accepting you on F1 or J1 then THEY WILL sponsor you by issuing form I-20(in case of F1) & DS-2019(incase of J1). Comments: They have a big University Hospital and a Vetrans (VA) (means millitary hospital). F1 Visa. The Department of Surgery appears to be flexible in considering & accepting B1/B2 visa for electives. They have a special VISSA Surgery program that is ONLY offered to International students, not even to American students, for extended period(3-6months). Only the surgical program does NOT require USMLE Step 1, Other departments DO.
