University list based on USMLE Step1 & TOEFL Req

This webpage is created to provide you with a list of Medical Universities in United states based on whether or not they require USMLE step 1 as part of their eligibility requirement for electives.

I have classified the US medical schools into Six categories based on their requirements:

Group A: Institutes that will NOT accept you for an elective position if you have NOT taken USMLE Step 1 WITHOUT Exceptions

Group B: Institutes that do not require you to have taken USMLE Step 1 inorder to be elligible for electives

Group C: Institutes that require you to have taken USMLE Step 1 Or COMPLEX 1 (DO students), but can waive it off in specific situations or accept alternatives.

Group D: Institutes that neither require Step 1 nor TOEFL exam as requirements for elligibility of electives. Includes institutes that prefer TOEFL but dont require USMLE Step1 are included.

Group E: Institutes that require USMLE step 2 in addition to or without USMLE step 1 as part of USMLE requirements.

Group F: Institutes that require TOEFL only. But neither Step1 or Step 2

Group A: USMLE Step 1/Complex 1 Required With NO EXCEPTIONS

  3. Case Western Reserve University (CWRU)(Ohio)
  4. Wayne State University School of Medicine (Michighan)
  5. Memorial Solan Kettering Cancer center (New York)
  6. University of Texas Health Science Center San Antonio (UTHSCSA) (Texas)
  7. East Carolina University Brody School of medicine (North Carolina)
  8. State University New York (SUNY) Downstate (New York)
  9. University of Texas Medical school at Houston (UT-HSC)(texas)
  10. Mayo Clinic School of Graduate Medical Education(Minnesota/Florida/Arizona)
  11. Virginia Commonwealth University School of Medicine (Virginia)
  12. University of Cincinnati College of Medicine UCCOM (Ohio)
  13. Emory University School of medicine (Georgea)
  14. University of North Carolina (UNC) (North Carolina)

Group B: USMLE Step 1 NOT required for application
Note: These institutes may require TOEFL


  1. National Institute of Health (NIH) (Maryland)
  2. University of Connecticut (UConn)(Connecticut)
  3. Henry Ford Health system (Michighan)
  4. University of Texas Southwestern(UTSW) (dallas, texas)
  5. Maimonides Medical Center (New York)
  6. University of Texas Health Science Center San Antonio (UTHSCSA) (Texas) (Surgery only)
  7. New York University (NYU) (New York)
  8. University of California San Diego School of Medicine (UCSD)(California)
  9. University of Wisconsin School of Medicine(Wisconsin)
  10. Northwestern University Feinberg School of Medicine (Illinois)
  11. University of Kansas School of medicine.
  12. Yale school of Medicine (CT)(Connecticut)
  13. Harvard School of Medicine (MA)(massachusets)
  14. Tufts University(Massachusets)
  15. Johns Hopkins University School of Medicine (JHU) (Maryland) (Research only)
  16. North Shore-Long Island Jewish Health System

Group C: USMLE Step 1 Req BUT can Waive off or Accept Alternatives


  1. Mont Sinai School of Medicine (MSSM) (New York)
  2. University of North Carolina (UNC) (North Carolina) (can accept german Psykium/Step 2 as alternative but does not waive off)
  3. Thomas Jeffersson University (Pennsylvania) ( can accept local/home countary equvialant to USMLE Step 1 and TOEFL)

Group D: No USMLE Step 1 No TOEFL
Note: The list of these institutes may"prefer" TOEFL, but is not compulsory.

  2. University of Texas Southwestern(UTSW) (dallas, texas)

  4. New York University (NYU) (New York)
  5. Johns Hopkins University School of Medicine (JHU) (Maryland) (Research only)

  8. Harvard School of Medicine (MA)(massachusets)
  9. North Shore-Long Island Jewish Health System

Group E: USMLE Step 2 Req OR alternative to Step 1


  1. Cleveland Clinic (Florida)
  2. Mayo Clinic School of Graduate Medical Education(Florida)
  3. University of North Carolina (UNC) (North Carolina) ( If you have taken Step 1 or Paykium, you are NOT required to take Step 2, But if you have taken Step 2, but not Step 1 they will still consider you for application)

Group F: Institutes that require TOEFL only (no exception) NO STEP 1


  1. National Institute of Health (NIH) (Maryland)

  3. University of Wisconsin School of Medicine(Wisconsin)
  4. Northwestern University Feinberg School of Medicine (Illinois)
